Model Release & Liability Waiver Form

I hereby grant the photographers attending the event, Garden Sessions on July 12, 2025 in Lansdowne, PA  permission to photograph and/or record me during this event and to use those images and videos, either wholly or in part, in their portfolios, websites, social media accounts, blogs, contests, or editorial features. I understand and agree that these photographs and videos may be published and displayed in both digital and printed formats, including but not limited to Instagram, websites, printed portfolios, and online publications.

I grant the photographers a perpetual, non-exclusive, royalty-free license to use and reproduce the media captured of me from this event for the purposes of promoting their photography or creative services (e.g., portfolios, websites, social media, or advertising their own services). However, I do not grant permission for these images or videos to be sold, licensed to third parties, or otherwise used for commercial resale, product advertisements, or paid publications (such as magazines, stock photography platforms, or campaigns that generate income) without my prior written consent.

I understand and consent to the digital compositing, editing, or alteration of the images and videos, including changes to color, size, context, background, or any artistic modifications.

Rights and Restrictions

– The Photographer retains full copyright of the images they create.
– The Model may use the images in their possession to promote their modeling business (e.g., portfolios, exhibitions, social media, contests), but may not sell or license the photos or submit them for publication without the Photographer’s written consent.
– The Model agrees to credit the Photographer when sharing the images publicly on social media, websites, or other platforms.
– The Photographer is encouraged to credit the Model when sharing images, and the Model may be identified by name, provided they have given express permission to do so.
– The images must not be altered or distorted in a way that misrepresents the work or harms the reputation of either party.

Liability Release & Assumption of Risk

I acknowledge and agree to the following:

1. I am voluntarily participating in this event organized by Art Imagined Photography, and I fully assume all risks involved, including but not limited to injury, illness, or property damage resulting from attendance, participation, or travel to and from the event.
2. I waive and release Art Imagined Photography, the event organizers, all attending photographers, volunteers, venue owners, and any associated parties from any and all claims, demands, or liability for personal injury, property damage, or death, whether caused by negligence or otherwise, except in cases of gross negligence or intentional misconduct.
3. I indemnify and hold harmless the above-named parties from any claims arising from my participation in the event. I acknowledge that flash photography and strobe lighting may be used during this event and that I may be recorded or photographed in group or candid settings.

This is a community-based collaborative group, not as a professional production company, talent agency, or employer. All participants—including photographers, models, and guests—attend voluntarily and are responsible for their own safety, equipment, and transportation. Art Imagined Photography does not provide insurance or assume liability for any personal injury, loss, or damage.

I understand that this agreement shall be interpreted as broadly as permitted by applicable law and that it is binding upon me and my heirs, legal representatives, and assigns.

Any disputes arising from this agreement shall first be attempted to be resolved through good-faith negotiation or mediation. If such resolution is not possible, the matter shall be handled in accordance with the laws of the Commonwealth of Pennsylvania, and any legal proceedings must be filed in that jurisdiction.

I certify that I am over the age of 18 and have the legal right to enter into this agreement. I have read this document in full and understand that I am giving up certain legal rights. I agree to sign it voluntarily and of my own free will.
